Overview

Human umbilical cord mesenchymal stem cell (hUC-MSC) exosomes are extracellular vesicles derived from hUC-MSCs that contain a cargo of bioactive molecules, including proteins, lipids, and various RNA species (such as miRNAs). Developed by the Sixth Affiliated Hospital, Sun Yat-sen University, these exosomes are being investigated as a cell-free regenerative therapy for intrauterine adhesions (IUA) and thin endometrium. The therapeutic approach involves intrauterine perfusion of the exosomes following hysteroscopic surgery to promote endometrial repair. The mechanism of action is believed to involve paracrine signaling that modulates the local uterine environment, stimulates endometrial cell proliferation, enhances angiogenesis, and reduces subendometrial fibrosis. This modality aims to restore uterine cavity morphology and improve endometrial receptivity for embryo implantation, offering a potentially safer and more standardized alternative to direct stem cell transplantation.
