Overview

Human urinary kallidinogenase is a glycoprotein and serine protease extracted from human urine. It acts by converting low-molecular-weight kininogen into kinin, thereby activating the kallikrein–kinin system. This leads to increased levels of kinins, which promote vasodilation, angiogenesis, neuroprotection, and improved cerebral blood flow. Human urinary kallidinogenase has been primarily developed and approved in China for the treatment of acute ischemic stroke. Clinical studies have shown that it improves neurological function and outcomes in stroke patients with an acceptable safety profile. Its mechanisms include promoting angiogenesis (via upregulation of VEGF and apelin), enhancing cerebral perfusion, suppressing inflammation, inhibiting neuronal apoptosis, supporting nerve fiber regeneration, and reducing oxidative stress[1][2][3][5][6][8].
