Human vaccinia virus immune globulin is a purified gamma globulin (IgG) derived from pooled plasma of healthy donors previously vaccinated with live vaccinia virus vaccine and possessing high titers of anti-vaccinia antibodies. It is administered intravenously and provides passive immunity by supplying neutralizing antibodies against the vaccinia virus. The drug is FDA-approved for the treatment of complications resulting from smallpox vaccination—including eczema vaccinatum, progressive vaccinia, severe generalized vaccina, and certain aberrant infections induced by the vaccinia virus—especially in individuals with skin conditions or immunodeficiencies. It may also be used under expanded access for post-exposure prophylaxis or treatment of orthopoxvirus infections such as mpox. The mechanism involves direct neutralization of the virus via supplied antibodies; however, the precise immunological mechanisms are not fully characterized[1][3][8].
