Hydrangea paniculata extract is a botanical drug candidate derived from the stems of *Hydrangea paniculata* Siebold, a traditional Chinese medicinal plant. The extract is standardized to contain approximately 75-80% coumarin glycosides, primarily skimmin and apiosylskimmin. It is currently in preclinical development as a Class 1.3 natural medicine (or Class 5 herbal drug) for the treatment of chronic kidney disease (CKD), including membranous nephropathy and diabetic nephropathy. The drug exerts its therapeutic effects through multiple mechanisms: it activates the Nrf2 anti-oxidation pathway, inhibits Smad2/3-mediated profibrosis, suppresses NF-κB-mediated inflammation, and inhibits complement activation. Additionally, it exhibits a bidirectional interaction with the gut microbiota, where it restores microbial diversity and reduces the production of uremic toxins (such as indole and p-cresol) while increasing short-chain fatty acid production. A key feature of its pharmacology is the biotransformation of its parent coumarins by gut bacteria into the more bioactive metabolite 7-hydroxycoumarin (7-HC), which accumulates in the kidneys to provide renal protection.
