hydrocortisone + tofacitinib

A combination of hydrocortisone, a glucocorticoid corticosteroid, and tofacitinib, a Janus kinase (JAK) inhibitor. Hydrocortisone acts by binding to the glucocorticoid receptor, inhibiting inflammatory pathways including phospholipase A2 and NF-kappa B. Tofacitinib works by inhibiting JAK1 and JAK3, preventing phosphorylation and activation of STATs, thereby decreasing T-cell activation and pro-inflammatory cytokine production. When used together, they may have synergistic immunosuppressive effects, potentially increasing efficacy in inflammatory conditions but also increasing the risk of infections, particularly herpes zoster.
