Hydrocortisone + bacitracin zinc + neomycin sulfate + polymyxin b is a prescription **ophthalmic combination ointment** composed of three topical antibiotics and a corticosteroid. It is used for **steroid-responsive inflammatory ocular conditions** when bacterial infection is present or there is a significant risk of bacterial infection, including inflammatory conditions of the conjunctiva, cornea, and anterior segment of the globe, chronic anterior uveitis, and corneal injury from chemical, radiation, or thermal burns or foreign bodies. Hydrocortisone acts through the **glucocorticoid receptor** to suppress ocular inflammation, while bacitracin zinc inhibits bacterial cell wall synthesis, neomycin inhibits bacterial protein synthesis at the ribosome, and polymyxin b disrupts the bacterial cell envelope of susceptible organisms, together providing broad antibacterial coverage against common ocular pathogens such as *Staphylococcus aureus*, *Streptococcus pneumoniae*, *Escherichia coli*, *Haemophilus influenzae*, *Klebsiella/Enterobacter* species, *Neisseria* species, and *Pseudomonas aeruginosa*. The product is available as generic ophthalmic ointment and has historically been marketed under the Cortisporin ophthalmic brand family.
