Hydrocortisone + neomycin sulfate + polymyxin b is a fixed-dose **small-molecule combination** anti-infective and anti-inflammatory drug used primarily as **otic** and **ophthalmic** topical therapy. The product combines **hydrocortisone**, a glucocorticoid receptor agonist that suppresses local inflammation, edema, erythema, and pruritus, with **neomycin**, an aminoglycoside antibacterial that inhibits bacterial protein synthesis through binding to the 30S ribosomal subunit, and **polymyxin B**, a polypeptide antibacterial that disrupts the outer and cytoplasmic membranes of susceptible Gram-negative bacteria through interaction with bacterial lipopolysaccharides and phospholipids. It is marketed under brands including Cortisporin, Otosporin, AK-Spore HC, and Pediotic, and is approved for superficial bacterial infections of the external auditory canal, infections of mastoidectomy and fenestration cavities, and steroid-responsive inflammatory ocular conditions where bacterial infection or risk of bacterial infection is present, including conjunctival, corneal, and anterior segment inflammation and chronic anterior uveitis.
