Terra-Cortril (hydrocortisone + oxytetracycline + polymyxin B) is a topical combination medication consisting of a corticosteroid and two antibiotics. Hydrocortisone is a glucocorticoid that provides anti-inflammatory, anti-pruritic, and vasoconstrictive actions by binding to the glucocorticoid receptor. Oxytetracycline is a broad-spectrum tetracycline antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit. Polymyxin B is a polypeptide antibiotic that increases the permeability of the bacterial cell membrane, primarily targeting gram-negative organisms. This combination is classically indicated for the treatment of corticosteroid-responsive inflammatory conditions of the eye and ear where a bacterial infection is present. It is also being investigated as a topical treatment for pilonidal sinus wounds and has been used off-label in dentistry for conditions like dry socket.
