Hydrocortisone + polymyxin b sulfate + bacitracin zinc + neomycin sulfate is a marketed ophthalmic combination drug consisting of a corticosteroid plus three antibacterial agents, formulated as an ophthalmic ointment for steroid-responsive inflammatory ocular conditions when bacterial infection is present or there is a significant risk of infection. Hydrocortisone provides local anti-inflammatory activity through glucocorticoid receptor signaling, while neomycin and polymyxin B provide broad antibacterial coverage against susceptible gram-positive and gram-negative ocular pathogens and bacitracin adds activity particularly against susceptible gram-positive bacteria. The combination has been used for inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe, including chronic anterior uveitis and corneal injury from chemical, radiation, or thermal burns or foreign body penetration. It is distributed as a generic ophthalmic ointment and has been manufactured or marketed by Bausch & Lomb and generic ophthalmic manufacturers.
