The combination "hydrocortisone + prednisolone" refers to two separate corticosteroid medications that are sometimes used together or interchangeably in clinical practice. Both are glucocorticoids with anti-inflammatory properties, though they differ in potency, duration of action, and specific applications. ## Pharmacological Comparison Hydrocortisone has a 1:1 ratio for glucocorticoid (anti-inflammatory) and mineralocorticoid (water retention) properties, while prednisolone has a 4:0.8 ratio, making it more potent in terms of anti-inflammatory effects with less mineralocorticoid activity. In terms of potency, 5mg of oral prednisolone is generally considered equipotent to 20mg of oral or IV hydrocortisone, though some evidence suggests prednisolone may be six to eight times more potent than hydrocortisone at replacement doses. Regarding duration of action, prednisolone has a longer effect (18-36 hours) compared to hydrocortisone (8-12 hours). This difference in duration means hydrocortisone may need to be administered more frequently to maintain the same anti-inflammatory effect. ## Clinical Applications Both medications are used to treat various conditions including: - Inflammatory disorders - Allergic reactions - Autoimmune diseases - Adrenal insufficiency - Certain types of arthritis - Severe asthma For severe asthma, research has shown that intravenous hydrocortisone provides no additional benefit when given alongside oral prednisolone and bronchodilator therapy in patients admitted to hospital without ventilatory failure. When switching between these medications, care must be taken due to their different potencies and durations of action. Patients are typically switched from IV hydrocortisone to oral prednisolone as soon as possible, sometimes at a higher dose initially before weaning back to baseline.
