Hydromorphinol (14-hydroxydihydromorphine) is an experimental semi-synthetic opioid agonist and a derivative of morphine. Structurally, it is characterized by the hydroxylation of the 14-position and the saturation of the 7,8-double bond of the morphine skeleton. It acts primarily as an agonist at the mu-opioid receptor, providing potent analgesic and sedative effects. Compared to morphine, hydromorphinol is approximately twice as potent, exhibits a steeper dose-response curve, and possesses a longer duration of action. Although it was developed for pain management and historically used in medicine as bitartrate or hydrochloride salts (distributed under the trade name Numorphan in some regions), it is currently classified as a Schedule I controlled substance in the United States and is regulated under the Single Convention on Narcotic Drugs. There is currently limited or no commercial development for this compound.
