Overview

Hydroxocobalamin is a synthetic injectable form of vitamin B12 (cobalamin) used primarily to treat vitamin B12 deficiency and as an antidote for acute cyanide poisoning. It acts as a precursor to the active coenzyme forms of cobalamin—methylcobalamin and adenosylcobalamin—which are essential for DNA synthesis and cellular energy production. In cyanide toxicity, it binds cyanide ions to form cyanocobalamin (vitamin B12), which is then excreted in urine. Its main indications include pernicious anemia due to malabsorption or intrinsic factor deficiency and emergency treatment of cyanide poisoning[1][2][3][4][5][6].
