Hydroxychloroquine + abemaciclib + endocrine therapy is a combination therapy regimen investigated by the Medical College of Wisconsin for the treatment of advanced solid tumors and hormone receptor-positive (HR+), HER2-negative advanced breast cancer. The regimen combines hydroxychloroquine (an autophagy inhibitor), abemaciclib (a CDK4/6 inhibitor), and endocrine therapy (either aromatase inhibitors like letrozole and anastrozole or the estrogen receptor antagonist fulvestrant). The therapeutic rationale is based on the hypothesis that abemaciclib synergizes with hydroxychloroquine-mediated inhibition of autophagy to promote apoptosis and overcome resistance in tumor cells. A Phase 1 clinical trial (NCT04316169) was initiated to evaluate the safety and efficacy of this combination but was subsequently withdrawn in December 2022.
