Overview

Hydroxychloroquine + carfilzomib + dexamethasone is a combination therapy being investigated by the Norwegian University of Science and Technology (NTNU) for the treatment of relapsed or refractory multiple myeloma. This regimen combines hydroxychloroquine, a small molecule autophagy inhibitor, with the proteasome inhibitor carfilzomib and the corticosteroid dexamethasone. The therapeutic rationale is based on the observation that multiple myeloma cells, which produce high levels of monoclonal proteins, are sensitive to proteasome inhibition. However, these cells can develop resistance by upregulating autophagy as an alternative protein degradation pathway. By inhibiting autophagy with hydroxychloroquine, the regimen aims to block this escape mechanism, thereby enhancing the efficacy of carfilzomib and potentially re-sensitizing resistant myeloma cells. A Phase 1 trial is currently evaluating the safety and maximum tolerated dose of this triple combination.
