The x-MAP regimen is an experimental combination therapy comprising hydroxychloroquine, methotrexate, capecitabine, and bevacizumab, specifically developed for the treatment of refractory metastatic colorectal cancer (mCRC) with RAS (KRAS or NRAS) mutations. This regimen, investigated by Sergey Orlov, MD, utilizes hydroxychloroquine as an autophagy inhibitor to potentially overcome chemoresistance. Methotrexate and capecitabine serve as antimetabolites that disrupt DNA synthesis, while bevacizumab provides anti-angiogenic activity by targeting VEGF. The combination is currently being evaluated in a Phase 2 clinical trial to compare its efficacy and safety against regorafenib in patients who have failed standard therapies.
