Overview

Hydroxychloroquine + SUBA-itraconazole is a combination therapy being investigated by St Vincent's Hospital, Sydney, primarily in the HITMAN-PC Phase I/II clinical trial (NCT03513211). This regimen is designed for men with biochemically recurrent (BCR) prostate cancer to delay the initiation of androgen deprivation therapy (ADT). Hydroxychloroquine, a 4-aminoquinoline, acts as an autophagy inhibitor by increasing lysosomal pH and preventing the fusion of autophagosomes with lysosomes. SUBA-itraconazole is a proprietary 'Super BioAvailability' formulation of the antifungal itraconazole, which exhibits multi-modal anti-cancer activity, including inhibition of the Hedgehog signaling pathway via Smoothened (SMO) antagonism, inhibition of angiogenesis via VEGFR2, and disruption of cholesterol trafficking through the NPC1 transporter. Together, the combination targets lysosomal homeostasis and cellular survival pathways that are often upregulated in advanced prostate cancer.
