Overview

Hydroxychloroquine + sulfasalazine is a combination of two disease-modifying antirheumatic drugs (DMARDs) used primarily in the management of rheumatoid arthritis. Hydroxychloroquine is an antimalarial agent with immunomodulatory properties, while sulfasalazine is a sulfa drug that also acts as an anti-inflammatory and immunomodulator. Both drugs are used individually or as part of combination regimens to reduce inflammation, pain, and joint damage in autoimmune diseases such as rheumatoid arthritis. The mechanism of action for hydroxychloroquine involves inhibition of toll-like receptor signaling and interference with antigen presentation, leading to reduced activation of the immune system[6]. Sulfasalazine’s mechanism includes inhibition of prostaglandin synthesis via cyclooxygenase inhibition, suppression of various inflammatory mediators (including nuclear factor kappa-B), and possible folate depletion[8][10]. While both agents are effective DMARDs on their own or in triple therapy (with methotrexate), evidence suggests that the double combination does not provide significant benefit over monotherapy with either drug for rheumatoid arthritis[1][7].
