Hydroxyethylcellulose (HEC) gel is a pharmacologically inert, isotonic hydrogel derived from cellulose. In the context of HIV prevention research, it is widely utilized as the "universal placebo" in clinical trials for topical microbicides. Developed to be chemically stable and non-toxic to vaginal flora and the epithelial lining, it serves as a standardized control to evaluate the safety and efficacy of active microbicidal agents. Biosyn and other developers have employed HEC gel in Phase 1 imaging studies using enhanced MRI techniques to characterize the vaginal distribution, coverage, and retention of topical formulations before and after simulated coitus, providing critical data for the optimization of drug delivery systems in the female reproductive tract.
