Hydroxypipamperone is a small molecule butyrophenone derivative and the primary active metabolite of the antipsychotic pipamperone. Developed by Zhejiang Huahai Pharmaceutical, it is currently in Phase II clinical development in China for the treatment of major depressive disorder (MDD). Like its parent compound, hydroxypipamperone is characterized by its potent antagonism of 5-hydroxytryptamine receptor 2A (5-HT2A) and moderate antagonism of dopamine D2 receptors. This pharmacological profile suggests potential efficacy in treating depressive symptoms with a lower risk of extrapyramidal side effects compared to traditional antipsychotics. It is administered orally as a hydrochloride salt formulation.
