Hydroxyprogesterone caproate is a synthetic progestin (progestational steroid ester) used primarily to reduce the risk of preterm birth in pregnant women with a history of spontaneous singleton preterm birth. It acts as an agonist at the progesterone receptor, transforming proliferative endometrium into secretory endometrium, inducing mammary gland duct development, and inhibiting gonadotropin production or release. Hydroxyprogesterone caproate is administered by intramuscular injection and has been marketed under various brand names worldwide. It was originally introduced in the mid-twentieth century and reintroduced in the US as Makena before being withdrawn from the US market in 2023 due to regulatory decisions regarding efficacy[1][2][5][6]. The drug is metabolized mainly by CYP3A4/5 enzymes and excreted via urine and feces[2]. Its primary indication has been for prevention of recurrent preterm birth; it has also seen use historically for gynecological disorders and as part of combination therapies[5].
