Overview

Hyperbaric bupivacaine + isobaric bupivacaine is a combination of two forms of bupivacaine used for spinal anesthesia. Bupivacaine is an amide-type local anesthetic available in two distinct formulations that differ in baricity: hyperbaric bupivacaine (containing dextrose, making it denser than cerebrospinal fluid) and isobaric bupivacaine (dextrose-free, with density similar to cerebrospinal fluid)[1][2]. When administered together, either sequentially or as a mixture, these formulations provide unique benefits compared to using either form alone. The combination appears to offer advantages including: - Greater hemodynamic stability with less hypotension - Extended duration of both sensory and motor blockade - Balanced diffusion characteristics that combine the rapid onset of hyperbaric bupivacaine with the prolonged effect of isobaric bupivacaine[2][5][6] This combination has been studied particularly for cesarean sections and lower abdominal surgeries, where maintaining stable hemodynamics while providing adequate anesthesia is crucial[2][5][7].
