Hyperimmune intravenous immunoglobulin (hyperimmune IVIG or HIG) is a specialized form of intravenous immunoglobulin prepared from the plasma of donors with high titers of antibodies against specific pathogens or antigens. Unlike standard IVIG, which is pooled from the general population and contains a broad spectrum of antibodies, hyperimmune IVIG is enriched for antibodies targeting particular infectious agents (such as cytomegalovirus [CMV], hepatitis B virus [HBV], rabies virus, tetanus toxin, etc.). The product typically contains at least fivefold higher antibody titers to the target antigen compared to regular IVIG. Hyperimmune IVIG works by providing passive immunity—neutralizing pathogens directly and modulating immune responses through interactions with various components of the immune system including cytokines, complement proteins, Fc receptors on immune cells (B and T lymphocytes), dendritic cells, and more. It can be used for prophylaxis or treatment in settings where rapid immunity is needed or when vaccines are not available/effective[9]. Indications include prevention/treatment of infections such as CMV in transplant recipients (CMV-IVIG), hepatitis B exposure (HBV-IVIG), rabies exposure (rabies IGIV), tetanus exposure (tetanus IGIV), among others.
