The hyperinsulinemic euglycemic clamp is a clinical procedure traditionally used to measure insulin sensitivity, but in this context, it is being investigated as a therapeutic intervention to reduce surgical stress and postoperative inflammation in patients undergoing curative colorectal cancer resection. The procedure involves the intravenous administration of a high-dose bolus and subsequent infusion of insulin (Human regular insulin) to achieve hyperinsulinemia. To prevent hypoglycemia, a simultaneous infusion of dextrose (glucose) is administered, with the rate adjusted to maintain normal blood glucose levels (euglycemia). This metabolic intervention is hypothesized to exert anti-inflammatory and immunomodulatory effects by activating insulin receptors, potentially improving postoperative morbidity and long-term survival outcomes. The specific application for colorectal cancer is being developed and investigated by King Saud University.
