Overview

The hyperinsulinemic euglycemic clamp is a sophisticated clinical research protocol, often considered the gold standard for measuring insulin sensitivity in vivo. This specific iteration, utilized by Vanderbilt University, involves the simultaneous intravenous infusion of insulin to achieve a steady-state hyperinsulinemic environment, somatostatin to suppress endogenous pancreatic hormone secretion, and glucagon to maintain basal levels. A variable infusion of 20% dextrose is titrated to maintain blood glucose within a euglycemic range. The rate of glucose infusion required to maintain stability (the glucose infusion rate or GIR) provides a direct quantitative measure of how sensitive the body's tissues are to insulin. It is primarily used in clinical trials to investigate the pathophysiology of Type 1 Diabetes, MODY2, and other metabolic disorders.
