Overview

This hyperosmolar saline irrigation solution is an investigational fluid developed by the University of Missouri-Columbia for use during arthroscopic knee surgery. It is prepared by adding 120 mL of 23.4% sodium chloride to a 3-liter bag of Lactated Ringer's solution, achieving an osmolarity of approximately 593-600 mOsm/L. The solution is designed to address the common issue of fluid extravasation into periarticular tissues caused by pressurized irrigation during surgery. By establishing an osmotic gradient, the solution helps retain fluid within the joint space and may even draw fluid out of the surrounding tissues, thereby reducing postoperative swelling and associated complications. Furthermore, research suggests that this hyperosmolar environment may have chondroprotective effects by mitigating inflammatory mediator production and proteoglycan loss in the articular cartilage and meniscus, potentially leading to reduced postoperative pain and decreased narcotic consumption.
