Hyperpolarized carbon-13 alpha-ketoglutarate (HP 13C-aKG) is an investigational metabolic imaging probe used in magnetic resonance (MR) spectroscopic imaging. It is specifically designed to non-invasively detect the metabolic activity of isocitrate dehydrogenase (IDH) mutations in glioma. In IDH-mutant cells, the enzyme reduces alpha-ketoglutarate to the oncometabolite 2-hydroxyglutarate (2-HG). By using hyperpolarization to enhance the carbon-13 signal, HP 13C-aKG allows for real-time monitoring of this conversion, providing a tool to characterize tumor burden and metabolic state. It is currently being evaluated in early-phase clinical trials led by investigators at the University of California, San Francisco, in collaboration with the National Cancer Institute.
