Hyperpolarized Pyruvate (13C) Injection is an investigational diagnostic metabolic imaging agent used in conjunction with magnetic resonance imaging (MRI) to assess cardiac metabolism. The agent consists of [1-13C]-pyruvic acid that has undergone dynamic nuclear polarization (DNP), a process that enhances the magnetic resonance signal of the carbon-13 nuclei by up to 100,000-fold. This allows for the real-time, non-invasive visualization of metabolic pathways within the myocardium. Specifically, it tracks the conversion of pyruvate into metabolites such as lactate and bicarbonate, mediated by the enzymes lactate dehydrogenase (LDH) and pyruvate dehydrogenase (PDH), respectively. In clinical studies led by Sunnybrook Health Sciences Centre, this agent is administered intravenously alongside standard gadolinium-based contrast agents to evaluate metabolic dysfunction in patients with cardiovascular conditions, including heart failure, hypertension, cardiac hypertrophy, and type 2 diabetes mellitus.
