Overview

Hypertonic saline nasal wash is a concentrated sodium chloride solution (specifically 1.8% NaCl in the Stanford University protocol) used for the management of chronic rhinosinusitis (CRS). Unlike standard isotonic saline (0.9%), the hypertonic formulation creates an osmotic pressure gradient that draws excess fluid out of the edematous nasal mucosa, thereby reducing swelling and improving nasal airflow. Additionally, it acts to decrease the viscosity of nasal secretions, facilitating the mechanical clearance of thickened mucus, allergens, and inflammatory mediators by the ciliary system. The solution is typically buffered to a physiological pH to minimize irritation and optimize patient tolerability during large-volume irrigation.
