Hypertonic sodium lactate is a concentrated crystalloid solution, typically formulated as a half-molar (0.5 M) solution, used for fluid resuscitation and the management of intracranial hypertension. It functions primarily as an osmotic agent, exerting high osmotic pressure that draws fluid from the interstitial and intracellular compartments into the intravascular space. This mechanism allows for rapid hemodynamic stabilization with significantly lower infusion volumes compared to isotonic solutions, thereby reducing the risk of fluid overload and tissue edema. Additionally, the lactate component serves as a non-glucose metabolic substrate for the heart and brain, which is particularly beneficial during states of shock or injury. Unlike large-volume saline resuscitation, hypertonic sodium lactate does not cause hyperchloremic acidosis. It has been investigated in clinical trials for pediatric dengue shock syndrome, post-cardiac surgery resuscitation, and traumatic brain injury.
