This results in a reversible blockade of nerve impulse transmission. The progression of anesthesia follows a predictable pattern related to nerve fiber diameter, myelination, and conduction velocity. The order of loss of nerve function is: (1) pain, (2) temperature, (3) touch, (4) proprioception, and (5) skeletal muscle tone[2]. ## Clinical Advantages Hypobaric bupivacaine offers several clinical advantages: - Minimal hemodynamic fluctuations, making it suitable for high-risk cases - Preferential sensory blockade with minimal motor involvement - Earlier recovery, ambulation, voiding, and discharge - Improved hemodynamic stability compared to hyperbaric solutions - Particularly beneficial in elderly patients or those with compromised cardiovascular function[3] A dose of 5-7.5 mg of bupivacaine (approximately 3-5 ml of 0.15% hypobaric solution) is typically sufficient for surgeries lasting 60-90 minutes[3].
