I-131-labeled anti-HMWA Fab is a radiolabeled antibody fragment specific for the high molecular weight antigen (HMWA), a tumor-associated proteoglycan expressed on human malignant melanoma cells. The drug consists of murine-derived Fab fragments—antigen-binding fragments of immunoglobulin G—chemically conjugated with radioactive iodine-131. The mechanism of action involves targeted binding of the Fab fragment to HMWA expressed on melanoma cells, thereby delivering cytotoxic ionizing radiation directly to the tumor site, allowing both radioimmunodetection (imaging) and radiotherapeutic effects. Clinical studies indicate tumor-selective localization and reduction in metastatic lesion size following administration. The modality avoids significant accumulation in the liver, offering improved tumor selectivity over other antibody targets for melanoma such as p97[1][8].
