i-Factor Peptide Enhanced Bone Graft is a biologic bone graft substitute developed by Cerapedics. It consists of a synthetic 15-amino acid peptide (P-15) that mimics the cell-binding domain of Type I collagen, adsorbed onto anorganic bone mineral (ABM) derived from bovine hydroxyapatite. The P-15 peptide acts as a ligand for alpha-2 beta-1 (α2β1) integrin receptors on osteogenic cells, promoting their attachment, proliferation, and differentiation. This mechanism facilitates cellular responses to enhance bone fusion. It is FDA-approved for single-level anterior cervical discectomy and fusion (ACDF) and is being investigated for broader applications such as adult spinal deformity in trials like the one conducted at Complejo Asistencial Universitario de León.
