Overview

Ibalizumab is a humanized monoclonal antibody used in combination with other antiretroviral agents for the treatment of multidrug-resistant HIV-1 infection in adults. It binds specifically to domain 2 of the CD4 receptor on CD4-positive lymphocytes, acting as a post-attachment inhibitor. By binding to this site, it prevents conformational changes required for HIV entry into host cells after gp120 attachment, thereby blocking interaction with CCR5 and CXCR4 co-receptors and inhibiting viral fusion and entry. Unlike some other anti-CD4 antibodies, ibalizumab does not cause immunosuppression because it does not interfere with MHC class II molecule binding or normal immune function. Ibalizumab was developed by TaiMed Biologics and Theratechnologies and is marketed under the brand name Trogarzo[1][5][6].
