Overview

Ibandronate is a third-generation, nitrogen-containing bisphosphonate used to prevent and treat osteoporosis in postmenopausal women. It works by binding to hydroxyapatite in bone and inhibiting osteoclast-mediated bone resorption, leading to increased bone mineral density and reduced risk of fractures. Its mechanism involves inhibition of the mevalonate pathway in osteoclasts, specifically targeting farnesyl pyrophosphate synthase, which disrupts essential protein prenylation required for osteoclast function and survival. Ibandronate is also used for the prevention of skeletal-related events in patients with metastatic bone disease from cancers such as breast cancer or multiple myeloma, and may be used to treat hypercalcemia associated with malignancy.
