Ibandronate + alfacalcidol + calcium is a combination therapy regimen studied by the Chinese University of Hong Kong for the management of steroid-induced osteoporosis in patients with systemic lupus erythematosus (SLE). Ibandronate is a nitrogen-containing bisphosphonate that inhibits bone resorption by suppressing osteoclast activity through the inhibition of farnesyl pyrophosphate (FPP) synthase in the mevalonate pathway. Alfacalcidol (1-hydroxyvitamin D3) is a vitamin D analogue that is rapidly converted in the liver to 1,25-dihydroxyvitamin D3 (calcitriol), which acts as an agonist at the vitamin D receptor to enhance calcium absorption and bone mineralization. Calcium is included as a mineral supplement to ensure adequate substrate for bone formation. This specific combination was evaluated in a Phase 4 clinical trial to assess its efficacy in improving bone mineral density and microarchitecture in SLE patients receiving long-term glucocorticoid therapy.
