Ibogaine + magnesium is a combination therapy consisting of the plant-derived psychoactive compound ibogaine and the mineral magnesium. Ibogaine is classified as an atypical psychedelic or oneirogen and acts on multiple neurotransmitter systems, including serotonin, dopamine, and glutamate pathways. It has been primarily studied for its potential in treating substance use disorders but has also shown promise in alleviating neuropsychiatric symptoms such as PTSD, depression, anxiety, and functional disability associated with traumatic brain injury (TBI). Magnesium is co-administered to mitigate ibogaine’s known risk of cardiac arrhythmias (notably QT prolongation), enhancing safety during treatment. The combination was evaluated in a prospective observational study (the MISTIC protocol) involving veterans with TBI and severe psychiatric symptoms. Results indicated significant improvements in functioning and reductions in PTSD, depression, and anxiety scores at one month post-treatment without serious adverse events[1][5][6][7]. Controlled clinical trials are needed to confirm efficacy.
