ibrutinib + fludarabine + pembrolizumab

ibrutinib + fludarabine + pembrolizumab is a combination regimen being evaluated in a Phase II clinical trial sponsored by the National Heart, Lung, and Blood Institute (NHLBI). The regimen targets high-risk or relapsed/refractory chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL). It combines ibrutinib, a small molecule inhibitor of Bruton's tyrosine kinase (BTK) that disrupts B-cell receptor signaling; fludarabine, a purine analog antimetabolite that interferes with DNA synthesis and provides lymphodepletion; and pembrolizumab, a humanized monoclonal antibody that inhibits the programmed cell death protein 1 (PD-1) receptor to restore T-cell mediated anti-tumor immune responses. This triplet approach aims to achieve deeper clinical responses and minimal residual disease (MRD) negativity by combining targeted therapy, chemotherapy-mediated immune modulation, and checkpoint inhibition.
