Overview

**Ibrutinib + nivolumab** is a combination regimen of two distinct pharmaceuticals: ibrutinib, a covalent small molecule inhibitor of Bruton tyrosine kinase (BTK), and nivolumab, a monoclonal antibody targeting programmed cell death protein 1 (PD-1). Ibrutinib modulates B-cell signaling, leading to reduced proliferation and survival of malignant B cells. Nivolumab acts as an immune checkpoint inhibitor, enhancing antitumor T-cell activity by blocking PD-1 and preventing negative regulation of T-cell function. This combination is under clinical investigation for relapsed or refractory central nervous system (CNS) lymphoma, diffuse large B-cell lymphoma (DLBCL) Richter transformation, and relapsed/refractory chronic lymphocytic leukemia (CLL). Early-phase clinical trials show promising response rates and a manageable safety profile. The rationale for combination therapy is based on potential immune synergy, wherein BTK inhibition modifies the tumor microenvironment, enhancing T-cell responses and potentially augmenting checkpoint blockade efficacy[1][2][3][4][5][6].
