This is a combination of three active pharmaceutical ingredients—ibuprofen, paracetamol (also known as acetaminophen), and dexamethasone—used together for multimodal analgesia, particularly in the management of acute postoperative pain such as after total hip arthroplasty. Ibuprofen is a non-steroidal anti-inflammatory drug (NSAID) that works by inhibiting cyclooxygenase enzymes (COX-1 and COX-2), reducing prostaglandin synthesis and thereby decreasing inflammation, pain, and fever. Paracetamol acts primarily in the central nervous system to inhibit prostaglandin synthesis via COX inhibition, providing analgesic and antipyretic effects with minimal anti-inflammatory action. Dexamethasone is a synthetic glucocorticoid corticosteroid that exerts potent anti-inflammatory effects by modulating gene expression through the glucocorticoid receptor, suppressing multiple inflammatory cytokines and mediators. The combination has been shown to reduce opioid consumption postoperatively compared to other regimens[1][2][3].
