Overview

Ibutilide is a class III antiarrhythmic drug used intravenously for the rapid conversion of atrial fibrillation and atrial flutter of recent onset to normal sinus rhythm. Its primary mechanism of action is the prolongation of cardiac action potential duration, mainly by blocking the rapid component of the delayed rectifier potassium current (I_Kr) and activating a slow inward sodium current, which increases cardiac refractoriness. Ibutilide is structurally related to sotalol and is administered as an intravenous solution due to poor oral bioavailability. It was developed by Pharmacia & Upjohn (now part of Pfizer) and is marketed under the brand name Corvert[1][5][7][8].
