Overview

Icalcaprant (formerly CVL-354 or ABBV-1354) is a small molecule antagonist of the kappa opioid receptor (KOR) being developed for the treatment of psychiatric disorders, including major depressive disorder (MDD) and bipolar depression. By selectively blocking KOR, the drug aims to modulate the dysregulated dopamine and dynorphin systems associated with mood disorders, potentially reducing symptoms of depression and anhedonia. Originally developed by Cerevel Therapeutics, the asset was integrated into AbbVie's neuroscience portfolio following its $8.7 billion acquisition of Cerevel in 2024. Clinical development includes Phase 2 trials for bipolar depression and Phase 1 trials for major depressive disorder, with additional studies evaluating target occupancy at both kappa and mu opioid receptors in the brain.
