ICAM1-MMAE is an antibody-drug conjugate (ADC) designed to target intercellular adhesion molecule-1 (ICAM1), a protein identified as being upregulated in ovarian cancer and expressed on both malignant epithelial cells and cancer-associated fibroblasts (CAFs). By targeting both the tumor cells and the supportive stroma, the drug aims to overcome stromal barriers and microenvironment-driven resistance that often limit the efficacy of epithelial-restricted therapies. The ADC consists of an ICAM1-targeting antibody conjugated to the microtubule inhibitor monomethyl auristatin E (MMAE). Preclinical studies have demonstrated that ICAM1-MMAE exhibits potent cytotoxicity against ovarian cancer cells and ICAM1-positive CAFs, leading to significant tumor regression and stromal remodeling, including the depletion of alpha-SMA-positive fibroblasts and reduced collagen deposition in the extracellular matrix.
