Icapamespib is a small molecule, orally bioavailable inhibitor of the heat shock protein 90 (HSP90), specifically targeting epichaperomes—pathological, stable chaperone complexes that form in diseased neurons and cancer cells. It is a purine scaffold analogue designed to cross the blood-brain barrier and selectively inhibit HSP90 when it is part of the epichaperome complex, leaving normal HSP90 function largely unaltered in healthy cells. Icapamespib has been investigated primarily for neurodegenerative diseases such as Alzheimer's disease and amyotrophic lateral sclerosis (ALS), with additional preclinical or terminated development in glioblastoma and other brain injuries. The drug was developed by Samus Therapeutics in collaboration with Memorial Sloan-Kettering Cancer Center[3][5][8].
