**ICE-T** is an opioid-sparing multimodal postoperative pain regimen developed by University Hospitals Cleveland Medical Center for women undergoing vaginal pelvic floor reconstructive surgery. The regimen combines around-the-clock topical ice packs, oral acetaminophen, and intravenous ketorolac, with opioid analgesia available for breakthrough pain when needed. Ketorolac provides nonselective cyclooxygenase inhibition to reduce prostaglandin-mediated pain and inflammation, while acetaminophen provides analgesia through incompletely defined predominantly central mechanisms; local cooling from ice packs may reduce pain and swelling. A multicenter randomized controlled trial reported lower inpatient narcotic use and improved pain scores at postoperative day 1 and 96 hours versus a standard opioid-based regimen.
