ICG318 + cyclophosphamide is a combination therapeutic regimen consisting of a compound chimeric antigen receptor (cCAR) T-cell therapy and a lymphodepleting conditioning agent, developed by iCell Gene Therapeutics for the treatment of refractory systemic lupus erythematosus (SLE) and lupus nephritis (LN). ICG318 is a multi-targeted CAR-T cell therapy designed to simultaneously target CD19 on B cells and B-cell maturation antigen (BCMA) on plasma cells and long-lived plasma cells (LLPCs), which are the primary drivers of autoantibody production in SLE. The therapy is uniquely armored with an IL-15/IL-15Rα sushi domain, which provides autocrine and paracrine stimulation to enhance T-cell expansion and persistence. This armoring allows for a simplified conditioning regimen using only cyclophosphamide, omitting the need for fludarabine and its associated toxicities. Clinical data have demonstrated deep immune resetting, leading to sustained medication-free complete remission and the elimination of elevated autoantibodies with a favorable safety profile.
