**ICH-1** is an investigational eight-herb traditional Chinese medicine formula developed for acute intracerebral hemorrhage. It includes the blood-activating and stasis-dispersing components **Hirudo** and **Tabanus** and was evaluated versus placebo and the related six-herb ICH-2 formula in the CRRICH randomized trial. In patients treated within 6 hours of symptom onset, ICH-1 did not significantly improve clinical outcomes and was associated with increased bleeding risk, plausibly related to its blood-activating herbal components.
