Overview

**ICOS-Fc fusion protein** is a recombinant soluble fusion protein consisting of the extracellular domain of human inducible T-cell co-stimulator (ICOS, CD278) fused to the Fc portion of human IgG1. It functions as a dual immunomodulatory agent, acting as an **antagonist of ICOS** on activated T cells and an **agonist of ICOS ligand (ICOSL, CD275)** on antigen-presenting and non-hematopoietic cells, thereby modulating cytokine release (e.g., reducing IL-1β, TNF-α, IL-6), inhibiting cell migration to inflamed tissues, and suppressing pro-inflammatory signaling pathways like FAK/p38 and NLRP3 inflammasome. Developed through research into the ICOS-ICOSL axis, it has demonstrated protective effects in preclinical models of sepsis by counteracting inflammation and organ dysfunction (liver, kidney), with potential applications in inflammatory, autoimmune, and possibly oncologic conditions due to its impact on T cell responses, dendritic cell function, and osteoclast activity.[1][5][9]
