Overview

Icosema is a once-weekly fixed-ratio combination of insulin icodec, a basal insulin analog with an ultra-long half-life, and semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist. Developed by Novo Nordisk, this combination therapy is designed to simplify the treatment of type 2 diabetes by providing both basal insulin and GLP-1 receptor agonist benefits in a single weekly subcutaneous injection. The mechanism of action involves the activation of the insulin receptor to regulate glucose metabolism and the activation of the GLP-1 receptor to stimulate glucose-dependent insulin secretion, suppress glucagon release, and slow gastric emptying. Icosema is currently being evaluated in the COMBINE Phase 3 clinical trial program, which compares its efficacy and safety to daily basal insulin regimens and other standard-of-care treatments in patients with inadequately controlled type 2 diabetes.
