ICP-248 + obinutuzumab is a combination therapy under clinical development for the treatment of mature B-cell malignancies, including chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL). ICP-248 (mesutoclax) is a novel, orally bioavailable selective inhibitor of B-cell lymphoma-2 (BCL2), a protein that regulates apoptosis; by inhibiting BCL2, ICP-248 restores apoptosis in tumor cells and inhibits cancer growth. Obinutuzumab is a humanized type II monoclonal antibody targeting CD20 on B cells, driving cell death through direct cytotoxicity and immune-mediated mechanisms. This combination targets two critical pathways in B-cell malignancies, aiming for deeper remissions, reduced resistant mutations, and improved outcomes. The developers have advanced this regimen to Phase III clinical trials as first-line therapy for CLL/SLL, with expansion to other hematologic malignancies[1][3][5][7][9].
