Overview

The Id-KLH-pulsed dendritic cell vaccine is an autologous therapeutic cancer vaccine designed to induce tumor-specific immune responses, primarily in B-cell malignancies such as multiple myeloma and B-cell lymphoma. It is prepared by isolating dendritic cells (DCs) from a patient's peripheral blood, then pulsing them ex vivo with the patient's tumor-derived idiotype (Id) protein (a unique, tumor-specific immunoglobulin) that has been conjugated to keyhole limpet hemocyanin (KLH), which acts as a carrier protein and immune adjuvant. These loaded DCs are re-infused into the patient, where they present the Id antigen to the immune system, aiming to stimulate both cellular (T cell-mediated) and humoral (B cell-mediated) anti-tumor immune responses. The vaccine targets clonal immunoglobulin expressed by malignant B cells. Trials have shown that it elicits idiotype-specific immune responses with some evidence of anti-tumor activity and is well tolerated. However, comparative studies in animal models have suggested that dendritic cell vaccines pulsed with tumor antigens are generally more effective than protein vaccines alone[1][3][5].
